...lagi liat-liat abstrak singkat beberapa jurnal Pediatrics dan menemukan artikel ini: Declining Diagnosis of Birth Asphyxia in California: 1991-2000. Satu studi kohort retrospektif yang mencari insidens dan tingkat keparahan kasus asphyxia pada rentang tahun 1991-2000. Studi kohort retrospektif adalah studi yang dilakukan dengan melihat sumber data (dalam hal ini catatan rumah sakit di Negara bagian California), pada rentang waktu yang dimulai dari titik waktu tertentu sampai titik waktu tertentu ke belakang (pada studi ini ditetapkan tahun 2000 sampai 10 tahun ke belakang, tahun 1991). Asphyxia disini adalah satu gejala kesulitan bernafas pada bayi neonatal yang ditandai dengan kulit bayi berubah warna menjadi biru, dapat terjadi pada 2 sampai 9 bayi neonatal per 1000 kelahiran hidup.
...begitu membaca kesimpulan studi ini, cuma ada 2 kalimat:
The diagnosis of birth asphyxia has decreased dramatically in recent years. The factors that are responsible for this decline are unclear and deserve additional investigation.
Ya ampuuuun. Jadi cuma begini doang? Studi segitu gedenya...sebentar, berapa banyak tadi sampelnya ya? Oh, ini dia: 5.364.663 kelahiran hidup! Gila kan??! Mereka memang menemukan penurunan kejadian asphyxia dari 14.8 menjadi 1.3 per 1000 kelahiran hidup, tapi apa yang membuat angkanya jadi turun dramatis –disitu ditulis: unclear! Nggak ada usaha laen apa??!

Our study is based on administrative data and thus is subject to a number of limitations. We were unable to determine the criteria used by treating physicians to make specific diagnoses such as birth asphyxia, and we relied on hospital discharge diagnoses that may not have been recorded accurately. Although we did not find evidence that incidence rates for birth asphyxia declined because of increased use of related codes, it is possible that other systematic changes in coding practice contributed to the apparent decline in birth asphyxia. However, the concept of birth asphyxia is problematic and subject to conflicting and potentially misleading interpretation that lead only to additional confusion among patients, clinicians, and researcher.
